mRESVIA (Respiratory Syncytial Virus Vaccine) Vaccine administration error- discovered that the mRESVIA vaccine (lot number 3041770) was administered after its published expiration date. Moderna, the vaccine manufacturer was contacted the vaccine administered on 12/18/25 from lot number 3041770 likely did not maintain the activity necessary to elicit an immune response against the respiratory syncytial virus (RSV) at time of administration Recommend patient receive a replacement dose of RSV vaccine to ensure patients are properly protected against RSV. Disclosure made to the patient and schedule a replacement dose
