Patient presented to the clinic for 4 vaccinations needed for nursing school entrance. Pt reported no previous poor reactions to vaccines, nor allergies to medications or foods. At 15:00PM, PHN prepared medications and administered an IM (TDAP) and SC (VAR) in the Right Arm, and IM (Hep B) and IM (FLU) in the Left Arm. Immediately after removing the needle for the Flu vaccine, the client began to bleed. PHN applied slight pressure to the Flu (lower of the 2 Deltoid sites) injection site to control bleeding, while cleaning the dripping blood off the client's arm with PHN's other hand. After about 10-15 seconds of pressure, PHN removed gauze to find a nickel-sized area of visible inflammation (no erythema noted). PHN applied small adhesive bandage to the area and assessed client. Pt reported no pain to the area, difficulty breathing, rapid heart rate. PHN informed client that I noticed some inflammation after the injection, and would like to observe him for a period of time to ensure no severe adverse reaction. PHN collected an ice pack for client to apply to injection site around 15:03. While observing, PHN checked respiratory status, with normal and consistent breaths and the ability to carry a conversation without SOB. Additionally, PHN assessed client's blood pressure at 15:14, which was 139/82 with a pulse of 56. Client noted he was unsure if he has high blood pressure, as he has not checked it in "a long time." After 15 minute waiting period, client continued to report no difficulty breathing or pain/itching. Client displayed no signs of SOB or severe adverse reaction. Nurse assessed the injection site prior to pt leaving clinic - swelling had reduced almost to its pre-injection state and no redness/heat noted at the site. Client felt comfortable leaving the clinic, walking out at 15:18 PM.
