Symptom narrative

patient came in for an appointment and was due for multiple vaccines, Tdap and Polio being some of them. Patient ended up receiving the Kinrix vaccine which is DTap and Polio instead. Intention was trying to reduce number of pokes for the visit but instead messed up the dose now leaving him with needing another dose of Tdap.
