Symptom narrative

It was noted by provider to administer HepA, the consent form prepped and signed for HepA by parent, but HPV was grabbed from the fridge. Concerned about how patient was going to respond to the vaccine, she had a history of intense responses while administering, the vaccine was not reviewed by another MA before entering the room.
