Edema and pain located on Rt bicep, shoulder, and arm pit area which began 1/7/26. Also reports 101 F fever on 1/8/26, headache, and loss of appetite that began around the same time. Alternating between Tylenol and IBU for symptom management with some relief noted with IBU. Denies difficulty breath, SOB, or chest pain. Encouraged warm compress to area and continue to alternate between Tylenol and Ibu. Per APRN, RTC for further evaluation of edema. Pt verbalized understanding. Scheduled to RTC, 1/8/26 at 1500. Pt reports swelling, red patches, and tenderness at injection site. Symptoms began 1/7/26. Also reports fatigue and body aches that began today, 1/8/26. Symptoms managed with IBU and cold compress to swelling with some relief noted. No pitting edema, drainage, or redness noted to Rt armpit. Slight swelling noted to Rt Deltoid. 3 quarter size red patches located on anterior side of deltoid. Quarter size redness noted at injection site on Rt Deltoid. Advised pt to continue to manage symptoms with IBU/Tylenol and warm/cold compress. RTC if symptoms worsens or persist. Advised when to seek emergent care such as uncontrolled fever or severe pain.
