Other Case identifier(s): MOD-2025-793029 (MODERNA), US-MODERNATX-MOC20260102000007 (MODERNA), MOC20260102000007 (MODERNA), US-MO-MOC20260102000007 (MODERNA). A patient (age and gender not provided) received BNT162b2 omicron (lp.8.1) (COMIRNATY (2025-2026 FORMULA)), on 26Dec2025 as dose 1, single (Lot number: NA4459, Expiration Date: Sep2026) intramuscular for covid-19 immunisation. The patient's relevant medical history and concomitant medications were not reported. Vaccination history included: Spikevax nos (dose 1), administration date: 2021, for COVID-19 vaccination; Spikevax nos (dose 2), administration date: 2022, for COVID-19 vaccination; Spikevax nos (dose 3), administration date: 2023, for COVID-19 vaccination. The following information was reported: HYPERTENSION (non-serious) with onset 26Dec2025, outcome "not recovered", described as "blood pressure was so high". The event "blood pressure was so high" required physician office visit. Relevant laboratory tests and procedures are available in the appropriate section. Therapeutic measures were taken as a result of hypertension. Additional information: The reporter contacted the company regarding side effects and later called back to clarify product information. The reporter stated that they initially assumed the vaccine administered was Moderna because they had always received Moderna COVID-19 vaccines since the start of the COVID-19 vaccination program, with previous doses administered in 2021, 2022, and 2023. After being asked to provide a lot number, the reporter accessed the patient portal and realized that the administered vaccine was not Moderna but Pfizer. Regarding symptoms, the reporter stated that the concern was related to elevated blood pressure. At the doctor's visit on 26Dec2025, the reporter's blood pressure was 138. By Monday, 29Dec2025, the reporter went to a different doctor, expecting blood pressure to be normal or only slightly elevated, which the reporter described as similar to white-coat hypertension. However, the blood pressure measured at that visit was approximately 220, which the reporter stated had never occurred previously. The reporter stated that their primary care provider later contacted the physician who had seen the reporter and expressed concern that the reporter had been allowed to leave with a blood pressure of 220 without intervention. The reporter was initially advised to monitor blood pressure and provide updates. The reporter monitored blood pressure at night using a home device and noted that values remained close to the 200s. On 30Dec2025, the reporter returned to the doctor, where blood pressure was taken two to three times and measured in the 190s and a little above on repeated readings. The reporter stated that the physician was concerned due to the persistently high readings. The reporter further stated that on the evening of 30Dec2025, they took a blood pressure medication for the first time. Per the physician's instructions, the reporter took losartan 50?mg, to be taken once daily. A follow-up appointment was scheduled for 07Jan2026. The reporter was advised not to measure blood pressure during the week prior to the follow-up visit, as frequent monitoring could cause additional stress. The reporter expressed concern that if the Pfizer COVID-19 vaccine caused the increase in blood pressure, they did not want to remain on blood pressure medication. The reporter also stated that they had contacted Pfizer and reiterated that they had always requested Moderna COVID-19 vaccines in the past and would obtain vaccination elsewhere if Moderna was not available.
