Patient presented to clinic for covid vaccine and received the vaccine without checking the expiration date, relying on the clinic's system to remove expired medication. While documenting, I discovered the vaccine was expired. The manager and patient were promptly notified and the patient was educated and advised to call if further clarification or assistance is /was needed. patient reported no adverse effects at of calling.
