No symptoms reported; Nurse reported that PROQUAD which underwent a temperature excursion was administered.; This spontaneous report was received from a Licensed Practical Nurse and referred to a 4-year-old patient of unknown gender. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 26-Dec-2025, the patient was vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) injection (0.5 ml, lot #Z013990, expiration date: 02-Mar-2027; strength and route were not reported) which underwent a temperature excursion for prophylaxis (Product storage error). The vaccine was reconstituted with sterile diluent (BAXTER Sterile Diluent) (lot #Z003252, expiration date: 05-Dec-2027). Temperature excursion was -14°C to -10°C for 44 hours 30 minutes 0 seconds; -10°C to 8°C (0C) for 2 hours 20 minutes 0 seconds. There was no previous temperature excursion. No symptoms were reported.
