Symptom narrative

No additional AE/No PQC; 15-year-old patient received PROQUAD; reported administration of improperly stored vaccines; This spontaneous report was received from an office manager and refers to a 15-year-old female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 21-OCT-2025, the patient was vaccinated with 1st dose of improperly stored (26°F, 3 hours 0 minutes 0 seconds, no previous excursions) measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) injection, administered intramuscularly in left deltoid, lot #Z005152, expiration date: 25-AUG-2026, for prophylaxis (exact dose was not reported) (Product storage error, Product administered to patient of inappropriate age). The vaccine was reconstituted with sterile diluent (lot# and expiration date were not reported). No additional adverse event was reported. This is one of two reports received from the same reporter.
