The patient's concurrent conditions, medical history and concomitant medications were not reported. On 05-Jan-2026, the patient was vaccinated with the first dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live lyophilisate and solvent for solution for injection (PROQUAD), 0.5 ml, subcutaneously, lot number: Z014047, expiration date: 03-Mar-2027 (anatomical location was not provided) for prophylaxis. The vaccine was reconstituted with sterile diluent (dose, lot # and expiry date were not provided). On an unknown date, the dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live lyophilisate and solvent for solution for injection (PROQUAD) experienced temperature excursion from 14 to 46 Fahrenheit degrees for 1 hour and 10 minutes and 6 to 14 Fahrenheit degrees for 1 hour.
