accidentally given Kinrix instead of Pediarix; This non-serious case was reported by a nurse via call center representative and described the occurrence of wrong vaccine administered in a 6-month-old male patient who received DTPa-IPV (Kinrix) (batch number 3C423, expiry date 06-SEP-2027) for prophylaxis. Co-suspect products included DTPa-HBV-IPV (Pediarix) for prophylaxis. On 22-DEC-2025, the patient received Kinrix. On an unknown date, the patient received Pediarix. On 22-DEC-2025, an unknown time after receiving Kinrix and not applicable after receiving Pediarix, the patient experienced wrong vaccine administered (Verbatim: accidentally given Kinrix instead of Pediarix). The outcome of the wrong vaccine administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 07-JAN-2026 Reporter reported had a child who was 6 months of age, who came in on 22-DEC-2025 for the well-child check, and he was inadvertently administered Kinrix instead of Pediarix. Reporter was calling to check if his/her understanding was correct, that the amount of DTaP and Polio in Kinrix was actually lower than in Pediarix. So, they could not consider this a valid dose, and they would have had to give Pediarix to complete the primary series.
