Symptom narrative

Patient was sitting at the time of vaccination, and advised to stay seated. Patient became pale and eventually lost consciousness. This RN aided patient to the floor to lay the patient flat on his back and provided a cushion under the patient's head. Patient regained consciousness quickly and legs were kept elevated. Vital signs were then recorded as the following: vitals at 1509 were blood pressure reading of 140/67 with HR of 90 and O2 98%. Patient vitals at 1515 were blood pressure reading of 149/78 with HR of 67 and O2 at 98%. Patient was then given crackers and soda. Patient was feeling drastically better after kept under close observation for at least 15 minutes and was able to walk to his vehicle.
