Symptom narrative

A vaccine administration error occurred in which the patient received two doses of Influenza High-Dose vaccine during the 2025?2026 influenza season. The first dose was administered on 09/17/2025. The second dose was administered on 10/15/2025. Administration of the second dose was unintentional and not clinically indicated, as only one dose of Influenza High-Dose vaccine is recommended per influenza season. The error was identified during subsequent review of immunization records. At the time of reporting, there have been no adverse reactions or symptoms reported by the patient related to receipt of the additional dose.
