Incorrect dose administered

Symptom narrative

atient's mother agreed to RSV antibody injection for patient during well child check. Provider instructed writer to give 100mg dose of RSV antibody. Administered injection along with other scheduled vaccinations as directed. When documenting injection, it was realized that patient did not meet criteria for 100mg dose but rather should have received 50mg based on weight between 8-9lbs. Per guidelines patients below 8mos and below 5kg are to receive 50mg and those above 5kg are to receive 100mg. The confusion may have came from thinking the patient was 8kg instead of 8lbs. Provider notified family of patient. No further treatment neccesary at this time.
