The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 28-Oct-2025, the patient was vaccinated with Rotavirus Vaccine, Live, Oral, Pentavalent (Rotateq) Oral solution at a dose of 2 mL administered orally (po) (valid lot# 2124042; expiration date: 20-Jun-2026) for prophylaxis. Medical assistant reported that the vaccine administered to patient was improperly stored (Product storage error). There was no additional AE (No adverse event).
