A medical assistant inadvertently administered this vaccine (Meningococcal B Bexsero) instead of the intended different meningococcal vaccine. The error was identified immediately during chart documentation. The patient?s parent was notified the same day, followed by a follow-up phone call from the office manager and physician. The patient reported localized arm soreness after vaccination. The physician determined this was an expected reaction to the vaccine and not an adverse response. No treatment was required, and the patient recovered without complication.
No medical tests or laboratory results were needed as a result of this error.
