Symptom narrative

On 12/17/2025 during system downtime , Provider order Spikevax 6 months-11 years for 6-month-old infant. Patient received a Moderna Spikevax 12y+ . On 01/09/2026, error was noted when MA was entering in system based on NDC number, both patient and MD were notified. Patient had telehealth appt on 01/09/2026 to address concern and provide reassurance.
