What the patient should have received is: DTAP + IPV (Kinrix) MMR Varicella Flu The issues I am finding are that: Instead of a Kinrix, it says a Quadracel was administered (which we do not have in stock), but the lot # is for a TDAP. The polio vaccine has the same TDAP lot # listed. Meaning the patient never received a DTAP or Polio, but an out of age range TDAP. The MMR vaccine has an MMRV lot # listed, suggesting pt received both Varicella and MMR. Which would be appropriate, but there is also a documented Varicella vaccine, but it has a lot# that doesn't correspond with any vaccine or diluent in stock. Suggesting either the pt received two Varicella vaccines, or just an MMRV, but cannot be sure. Flu shot seems to be administered and documented appropriately. What the chart suggests the patient received instead of what was required is: TDAP MMRV Potentially a second Varicella? In short, I cannot be sure what the patient got administered.

We will be ordering Tetanus, MMR, Varicella and polio titers to confirm immunity to see if any last doses in the series need to be administered. So far we have been unable to reach the parents, but will continue to reach out.
