Symptom narrative

Systemic: Confusion-Mild, Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Systemic: Seizure-Medium, Additional Details: After vaccine, as pt and mom were at the consultation window when pt went down face first (cheek). Pt seized for about 30 seconds and then came out of it. I provided water and ice to cool the pt down as she stated she was warm. We asked questions to determine if she was coherent and she answered them correctly but mom said she was not acting like herself. I let her know about the possibility of head trauma with the nature of the fall and the seizure. Mom decided to take her to the ER.
