Systemic: 2 high dose flu vaccines given-Mild, Additional Details: patient came in and stated she went to md office on 12/5/25 asking for a covid vaccine (she was given a flu shot unknown to her not covid) she came to Pharmacy asking for a flu shot and was given on 12/31/25. she got cob and noticed she received 2 flu shots and not a covid vaccine md stated Pharmacy should have looked and registry (not sure if done) they were supposed to give covid not flu. not having any issues
