Symptom narrative

Pt given flu injection on right deltoid. Tolerated fine. He stood to the side while sibling was getting their vaccine and within a minute, fainted and fell over. Father caught him and patient woke up within a second or two of falling. He did not hit his head. Patient looked pale, made gagging motions and coughs. Given water and emesis bag. Rapid response team called. Patient was laid down and elevated legs. Pt lung sounds clear, no rash. Pt coughing due to nausea without emesis. Following assessment, patient sat up without dizziness and returned to sitting in a chair.
