No additional AE; Nurse reported that a 30 year old patient received PROQUAD due to a human error.; This spontaneous report was received from a nurse and refers to 30-year-old patient of an unknown gender. The patient's medical, concurrent conditions and concomitant therapies were not reported. On 18-Dec-2025, the 30-year-old patient was vaccinated with the first dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (valid lot #Y019176, expiration date: 18-May-2026) as prophylaxis due to human error (product administered to patient of inappropriate age). The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported). No additional adverse event (AE) was reported.
