Incorrect vaccine administered. Adult RSV vaccine was given. No reported symptoms or signs reported. I have tried 3 times to contact the family to inform them of the error but have been unable to get ahold of them. They do no have voicemail set up on their phone. I will continue to call until I can speak to mom.
