Pediarix instead of Boostrix administration; Polio extra dose administration (4 polio doses in total: 2 Pentacel and 2 Pediarix); Pediarix administration out of the recommended age; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong vaccine administered in a 11-year-old female patient who received DTPa-HBV-IPV (Pediarix) (batch number Z9S4D, expiry date 04-JUN-2027) for prophylaxis. Co-suspect products included DTPa (Reduced antigen) (Boostrix) for prophylaxis. Previously administered products included Pentacel (received 2 doses of Pentacel on an unknown date) and Pediarix (received 1 dose of pediarix on an unknown date). On 05-JAN-2026, the patient received Pediarix. The patient did not receive Boostrix. On 05-JAN-2026, an unknown time after receiving Pediarix, the patient experienced wrong vaccine administered (Verbatim: Pediarix instead of Boostrix administration), extra dose administered (Verbatim: Polio extra dose administration (4 polio doses in total: 2 Pentacel and 2 Pediarix)) and inappropriate age at vaccine administration (Verbatim: Pediarix administration out of the recommended age). The outcome of the wrong vaccine administered, extra dose administered and inappropriate age at vaccine administration were not applicable. Additional Information: GSK receipt date 06-JAN-2026 The healthcare provider wanted to know how to proceed with the patient that received a dose of Pediarix (being the fourth that the patient receives in the polio immunization schedule) instead of Boostrix (that would have been the fourth in the tetanus, diphtheria, and pertussis immunization schedule). The reporter consented to follow up. The vaccine administration facility was the same as primary reporter. The batch number was not provided upon follow- up with the reporter.
