Patient came in for annual physical. Sticky note in patient?s chart stated patient was due for second dose of Shingrix. No one was available double check, so dose was given outside of double check. After review of patient?s chart, under the vaccine tab, patient had already received 2nd dose that was not previously reported in the sticky note so this dose was unnecessarily given. No treatment was given, patient reported nausea and vomiting a few times the next day but did go to work. Followed up again 3 days later and patient stated he was feeling fine.
