Symptom narrative

On 1/10/26 approximately 11:30am Mom said patient's left upper arm at vaccine site was tender, red, swollen and warm to touch. Patient was seen in our office on 1/12/26 and the injection site is noted to be tan/brown macular oval of 7x8 cm on left upper arm lateral surface. No tenderness. FROM of arm with no pain on flexion or extension.
