1/13/2026 3:01 PM pt here with mother for nurse visit appt for immunization HPV was given after about 2-3 mins pt had a syncope event to the floor Per LVN head injury was prevented due to mother holding pt. At the time I walked into the treatment room pt was lying down on the floor Was alert, oriented to her name and place. Pt was able to sit up and slowly stand up placed on to exam table and vital signs were Taken Dr was notified. Pt was given juice and crackers and will be monitored for the next 20 mins. At 3:32 PM pt completed juice and crackers alert and oriented, denies any symptoms at this time. Vitals signs rechecked consulted with dr pt to be discharge to home with mother. Pt walked out of clinic with mother with steady gait Mother had no other questions or concerns -RN
