Symptom narrative

Pt presents in clinic on 1/12/26 for a follow up, in which they wanted to receive the TDAP vaccine. Noticing the count of vaccines was off in our inventory I asked for assistance to check inventory. From that point, the vaccine was verified with another medical assistant. Vaccine was given to pt and they had left the clinic. When documenting the vaccination in pts charts the LOTS would not enter, that is when it was noticed that the pt did not receive TDAP but instead was administered DTAP-IPV. This vaccine was given by me, at roughly 3:30-4 pm. Medical assistant manager was notified immediately.

Bacterial prostatitis, BPH associated with nocturia, and Hypertriglyceridemia
