Symptom narrative

Moderna COVID-19 Spikevax 0.25mL was ordered by the patient's PCP. The incorrect vaccine was selected from the vaccine refrigerator by the clinical staff. Patient instead received Moderna COVID-19 Spikevax 0.5mL. Due to patient's age, this is the incorrect dose. The PCP was notified after administration that the child had received the wrong dose. Parent's were informed and counseled on the vaccine error and possible side effects the child may experience.
