Symptom narrative

Patient states upon receiving her Hepatitis A vaccine she felt significant pain during administration and right after. She noticed an episode of numbness and tingling to the right upper extremity shortly after administration however it quickly resolved.

X-rays performed at urgent care with no signs of fracture. Outcome of event: Member continues with reduced ROM of the right arm and underwent an injection by orthopedics and referred to physical therapy.
