Symptom narrative

Patient already received Prevnar 20 on 4/14/25 and should not need second dose. However, the vaccine administration information was not submitted to State Immunization Database Website via pharmacy system for that vaccine. Therefore, when the pharmacist reviewed past vaccination records on 1/12/26 on the state immunization website, they did not see the prior administration and a dose of Prevnar 20 was administered again. Patient experience no side effects and symptoms after receiving the inadvertent second dose.
