Symptom narrative

There was no adverse reaction reported by Patient, This is to report a vaccine error of mistakenly administering Measles, mumps rubella and varicella combo (ProQuad) on an 18 years old Pt when the guidelines states it should not be given to anyone who is 12 years of age and older. Called and inform CDC about this error and was inform that the vaccine is valid, provided it was not given within 28 days of another live vaccine

Lab data

Per IIS records MMR 3 doses with one that was invalid and Varicella 4 doses with 2 doses that were invalid. This dose given on 01/06/2026 is the Second ProQuad .
