On 01/09/2026 at 2:40pm, at Pediatric Office, my 2-year old received the incorrect vaccination. She received the flu shot rather than the intended Hepatitis A vaccine. My husband had explicitly declined the flu vaccination prior to the administration of the shot. At 5:25pm, we received a phone call from the office and the doctor informed us of the error. It was admitted that the error had occurred and our lack of consent to the flu shot was also acknowledged. She stated that an internal investigation into how this occurred is underway. An addendum has been added to record detailing the error and lack of consent. We were also instructed to monitor for symptoms and recommended to bring our daughter back to the office or go elsewhere to have the appropriate Hepatitis A vaccination administered. We followed up with an independent physician at the urgent care on 1/13/2026 around 7:00pm who assessed our daughter and found no current concerns with her condition. No significant adverse events have been observed to date and we are continuing to monitor. Additional vaccine specifics (exact brand, lot number, etc.) are pending from the provider and I will follow up if I receive them.
12/21/25-12/24/25 flu like symptoms (103 fever for 24 hours), muscle aches, fatigue, cough, and congestion. Cough and congestion lasted for several more days.

1/13/2026 at 7:00pm, an independent physician at the urgent care assessed my daughter for a 2nd opinion and found no concerning reactions at this time.
