Symptom narrative

Pain at time of injection. Increasing redness, swelling and pain starting 1/03/2026. Increased to severe swelling and redness 1/04/2026 with pain in shoulder and elbow, arm hot to the touch. Virtual visit with Insurance virtual care Physician 1/04/2026, directed to local ED for treatment including antibiotics and steriods. PA diagnosed Cellulitis of left arm evening of 1/04/2026 at Hospital. Prescribed Cephalexin 500mg 4x per day and Bactrim 800mg 2x per day 1/04/2026, first dose provided at ED () with adverse reaction that evening; Night terrors, night sweats, nightmares. 1/05/2026 Prescribed Cefdinir 300mg every 12 hours for 14 days, Doxycycline 100mg 2x per day for 14 days, Cetrizine 10mg 2x per day.

Medical history

ADHD, Asthma, history of Lyme Disease, Acid Reflux.

Other medications

Adderall XR, Vitamin D and K2, Vitamin B12, Vitamin B6, Zyrtec.

Allergies

Eggs, Red Bell Peppers.
