On 01/09/2026, my son, a 5 year old liver transplant recipient on chronic immunosuppression Tacrolimus and CellCept (Mycophenolate) was administered two strictly contraindicated live virus vaccines: MMR (Lot #Y017528) and Varicella (Lot #Z008082). A third vaccine, Inactivated Influenza (Lot #UT8779LA), was the only authorized immunization for this visit. Timeline and Error Details: Authorized Treatment: The visit was explicitly scheduled and requested for an "Inactivated Influenza Vaccine" only. Contraindication Alert: The patient's transplant status was documented on intake forms and in the EMR. Electronic records indicate a "Contraindication Check" was performed, but the live vaccine contraindication was manually bypassed or ignored by staff. Informed Consent Failure: Staff failed to verbally identify the vaccines prior to administration. The Vaccine Information Statements (VIS) were withheld until after the injections were completed, in violation of 42 U.S.C. § 300aa-26. Treatment and Outcome: The State Liver Transplant Team has been notified of the error and is currently determining the necessary plan of action for potential vaccine strain viral infection or organ rejection. Current status: stable/ monitoring during the 4?14 day incubation window.
