On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: child was given only the liquid diluent portion of the Penmenvy vaccine without reconsitituting it with the lyophilized powder.) and inappropriate dose of vaccine administered (Verbatim: Inappropriate dose of vaccine administered). Additional Information: GSK Receipt Date: 13-JAN-2026 The reporter reported three incidents in which a child (patient) had been given only the liquid diluent portion of the Penmenvy vaccine without reconstituting it with the lyophilized powder. The reporter knew the children (patient) had not received any of the MenACWY component and therefore would need to be vaccinated for MenACWY, but he/she wanted to know whether giving the diluent alone was equivalent to a dose of Bexsero. If the diluent alone would not count as a dose of MenB, how soon could a dose of Penmenvy be given. The batch number was not provided upon follow up with the reporter.
