Symptom narrative

Resident initially had an order for an updated 2025/2026 COVID vaccine but the order was placed "on hold" due to cold symptoms. The resident was given the vaccine but the vaccine was not appropriately documented and the resident was given a second vaccine in a 24 hour period. This error was immediately identified, resident placed on alert charting x 3 days. Resident reports congested cough and overall malaise. Vital signs are stable. Contacted pharmacy, per pharmacist, extend alert charting to 7 days but no further intervention needed.
