Symptom narrative

Patient received COVID 19 vaccine in the left deltoid muscle, then the pharmacist administered Flucelvax into left deltoid muscle 1 inch below the COVID 19 vaccine. Patient was alert for about 30 sends after the administration of the second shot and then lost consciousness for about 10 seconds. The parent and the pharmacist held the patient in chair and monitored breathing. The patient came and wag given cool water to drink. He complained of ringing in the ear, which later subsided. Patient was taken home by his parents. No further medical assistance was needed.
