I was covering a Provider while their MA went to lunch and Follow providers request of patient wanting the flu vaccine. I Walked into Patient's room and used 2 patient identifiers to verify it was the right patient. After mom confirmed the identifiers, I verified with mom that they were requesting a flu shot today for patient. Mom stated yes. I asked mom if patient was good with sitting still or if extra help was needed but mom explained that she was unsure and was willing to hold if needed. Patient and parent requested the flu vaccine to go in right arm. I explained that we could do that, but that mom would need to hold patient's hands and ensure patient does not move away. I did offer if patient wanted vaccine in the leg but both agreed they wanted it in patient's arm. Patient was upset and explained to mom that she did not want the vaccine and mom stated that she needed to get the vaccine today to help protect her from the flu. I asked mom once more if she was okay with holding hands and body before starting the process of administering vaccine. Mom stated yes and to not count when giving the vaccine. I informed patient that I was cleaning the area first and that it would be cold and then we would let it dry for a few seconds before administering. Patient stated understanding. I informed mom to make sure she was holding hands before administering and mom stated okay. After I saw mom was holding hands and body I went to administer vaccine in the right deltoid. Mom accidentally let patients left hand go, the needle poked a little through the skin before patient grabbed my hand holding the needle pulled it away and tried to poke me with the needle. I moved my left hand out of the way when mom yelled and grabbed patients' hand again and told patient to let go. After patient let go I safety locked the needle and moved it away. Patients mother asked If I had gotten poked and if I was okay and I informed her that I did not get poked and was okay. I explained that I wanted to make sure patient was okay due to the needle being moved after initial poke. I showed mom that there was a dot of blood from the poke and a tiny scratch that was pink but not bleeding, mom said she was sorry many times for letting go of hand and was not worried about poke and scratch on patient. I informed mom that if she wanted, we could have the provider take a look to at patients arm just to ensure patient was okay and to inform provider of incident. Mom stated that since she barely bled, she was fine with her not coming in and informed me that they did not want to do the flu after all due to concerns for my safety. I explained why we request for parents to hold on to patients' hand and how we offer extra help if needed or if parents are not comfortable with holding patients to avoid these incidents. Mom stated understanding and explained how sorry she was for letting go. Mom was embarrassed by the incident and requested to not do the vaccine and was ready to leave. I stated understanding and explained to mom that I do need to inform provider of incident and reported incident right away to provider and leader. Mom stated understanding and informed me she was sorry again before leaving.
