two Shingrix doses were administered to the same patient; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of accidental overdose in a 59-year-old female patient who received Herpes zoster (Shingrix) (batch number CH929, expiry date 07-JUL-2027) for prophylaxis. Concomitant products included Varicella zoster vaccine rgE (CHO) (Shingrix). On 14-JAN-2026, the patient received the 2nd dose of Shingrix. On 14-JAN-2026, an unknown time after receiving Shingrix, the patient experienced accidental overdose (Verbatim: two Shingrix doses were administered to the same patient). The outcome of the accidental overdose was not applicable. Additional Information: GSK Receipt Date: 14-JAN-2026 and 15-JAN-2026 Pharmacist called to report that two Shingrix doses were administered to the same patient. The pharmacist mentioned that a wife and husband went to the pharmacy for their first dose of the shingles vaccine. Two doses were withdrawn but were mistakenly administered to one patient instead of one dose to each.
