Patient went to pharmacy on 01/14/2026 asking for Gardasil 9 vaccine, he was offered also for others vaccines such as Covid-19, Flu and Hep A +B. Patient reject the Flu shot since he was already vaccinated for the Flu in 08/25. In 01/14/2026 patient was vaccinated for Gardasil and Twinrix (desired vaccines), but by mistake he was also vaccinated with Flu a vaccine that he was already vaccinated. At the moment of vaccination no adverse effects were reported. Pt. was reached on 01/15/2026 and 01/16/2026 to see how he was doing he reported feeling a little discomfort at the arm that he was vaccinated and tingling at some of the fingers of that arm.
No illnesses were documented at vaccine administration record at the time of vaccination .
Based in patient record he is currently using medication for the following conditions: anxiety, high cholesterol, high blood pressure, insomnia and hypothyroidism.
