Symptom narrative

Patient was seen on 1/15/2026 and the provider ordered Vaxelis (Dtap/IPV/Hib/Hep B), Prevnar 20, and RotaTeq to be given. In error, the medical assistant administered Quadracel (Dtap/IPV) instead of Vaxelis. Provider spoke with patient's parents the next day, 1/16/2026, who stated that the patient is doing well besides some irritability.
