Symptom narrative

Immediately felt extreme pain when receiving TDAP shot in the ER for a burn injury to my hand. I immediately lost good use of my arm. I saw urgent care, my PCP, and ortho urgent care for the pain. SIRVA has been confirmed by MRI and phsycian's diagnosis.
