Mild redness to patient's left upper arm. The redness was approximately 7cm wide by 4cm tall. No warmth or tenderness. Full ROM of the extremity is intact with good pulses and brisk cap refill. The redness occurred below the axillary line and above the elbow. It has not worsened since onset but just has not improved. Discussed watchful waiting. Can take zyrtec oral PRN OTC if needed for itching. Discussed signs of infection and if these develop or symptoms fail to improve/resolve within 7 additional days to follow up with primary care provider or seek higher level of care. Patient received this vaccine at the primary care office for [withheld] but came to a different facility for evaluation of the injection site.
