Symptom narrative

19 month old female was administered 4th dose of Pediarix vaccine. No symptoms or signs have been noticed. Communicated with manufacturer via phone call to report the adverse event and that an extra dose in the series was administered instead of the Infanrix vaccine. GSK stated that they have no adverse events for the age of patient with that vaccine administered in the series documented. They stated the the CDC deemed the 5th dose of Hep B not harmful. DTaP vaccine was administered correctly, so patient is still up to date in series on that. The patient will need another dose of IPV since 4tj dose was administered prematurely; this dose will be given at 4 years of age.
