Symptom narrative

About 24 hours after injection, patient began with erythema around injection site, fever to 101.8, general malaise. Rash progressed over the next 3 days to encompass whole bilateral LE to to lower waist sparing buttocks. Temps x 4 days total with Tmax 103.5. Patient very fussy, normal po intake and normal wet diapers. Lab evaluation notable to high ESR/CRP, negative respiratory viral panel, CBC normal. Patient was sensitive to touch to LE's. Resolved over time with hydration, Zyrtec and hydrocortisone ointment. Thought to be more viral but concern given temporally related to the flu vaccine.

Trisomy 21, global developmental delay, hypothyroidism

Other medications

levothyroxine 37.5mcg daily

Lab data

1/14/26- negative outpatient flu/COVID/RSV and strep testing; 1/16- CBC with normal WBC, slightly low Hgb to 10.2, otherwise normal. CMP notable for Na 129 with normal Na after NS bolus. ESR- 76 and CRP 166. Negative respiratory viral panel.
