On 1/07/2026 at 2pm. Contacted patient's mother regarding how patient was doing since injection. Patient's mother reported patient was cranky and not wanting to walk due to the soreness at injection site. No other sx's. Mother was informed to contact clinic if she notices anything else with the patient. Mother v/u.
No illnesses at the time of vaccination and up to one month prior. --Only thing during patients 1/6/26 ov is she has had an intermittent rash for the past month. Notes: Discussed with mother that rash is consistent with eczema with some scattered keratosis pilaris. Recommended Cetaphil Sensitive Body Wash, ALL Free & Clear detergent, and Vaseline Sensitive lotion. She voiced agreement.
