On an unknown date, the patient received Penmenvy. On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Patient was given only the liquid component) and inappropriate dose of vaccine administered (Verbatim: Patient was given only the liquid component).
