Symptom narrative

Per patient, was fine after vaccine however next day around 0930 began to feel like he was going to pass out and vomit. Had near syncopal episode, but laid down in recovery position and felt better. Felt a little "off" for the next few days. Had armpit pain per patient. Stated had been short of breath with activity.

Medical history

Non-Hodgkin's lymphoma, prediabetes, GERD, anxiety, chest pain, skin lesion, insomnia.

Patient informed this RN on 1/16/26, stated was feeling better, denied any symptoms at that time. Cardiology spoke with patient, stated earlier cardiology follow up not warranted. PCP made aware.
