Symptom narrative

Patient came to clinic requesting Hep B vaccine. Discussed this would be 3rd Hep B vaccine. Patient voiced "Yeah. I don't know the kind I had for the first one." Patient received Heplisav at Health Department on 12/18/2025 as 2nd dose of Hep B. Followed nursing protocol to complete series of Hep B with Heplisav due to unknown dose 1 for Hep B. Heplisav administered. Patient tolerated well. Patient instructed to wait on site for 15 minutes prior to leaving. Patient voiced no concerns.
